Medical/Health Declaration
Children are not allowed to start school until the medical/health declaration form has been completed.
Emergency Contact Information
It is important that this information is kept up to date. Please make sure that the School Office is informed of any change of address or phone number. If the contact is a friend, or neighbour, please ensure that your child is aware of who will be collecting them, in case of sickness or emergency.
In the case of an emergency, when a parent cannot be contacted, the school nurse will take the child to hospital. It is important that you sign the medical consent form when your child starts school.
Sick Children
If children are found to have diarrhoea, vomiting, fever, discharging eyes or an undiagnosed rash, they should be kept at home. This will enable them to recover more quickly and will help prevent the spread of infection amongst other children.
If your child has had medication before leaving home, please send a note stating the name of the medication, the time given and a contact telephone number for the day.
If your child requires regular medication, e.g. inhalers for asthma, please leave supplies with the nurse with a note stating the details of dosages.
Medical Services
The school nurse will administer first aid and attend to sick children. If it is felt that the child would be better at home, the nurse will contact the parents.
The school works closely with the Government Schools' Medical Team to organise regular medical inspections, routine inoculations and vaccinations. Sight and hearing tests are carried out regularly. Children's heads are checked regularly, but parents are advised that it is also their responsibility to check their child's head for lice.
Medical records are maintained by the school nurse and will be given to you when your child leaves the school.
